Patient Care Coordinator - PMS Central Pharmacy - Albuquerque, NM
Continued growth at VytlOne (formerly Maxor) has created an opportunity for a Patient Care Coordinator to join our growing PMS Central Pharmacy. This is an excellent opportunity for a licensed Pharmacy Technician professional with retail pharmacy or high volume experience to make a positive impact in the lives of the patients we care for, working with a devoted pharmacy operations team. This position is located in the PMS Central Pharmacy in Albuquerque, NM.

ESSENTIAL FUNCTIONS (These duties must be performed with or without reasonable accommodation. This document in no way states or implies that these are the only duties to be performed by the employee occupying this position):
1. Answers new referral intake and/or refill calls in a timely and professional manner.
2. Accurately inputs new referrals into the computer system and updates information in the computer system (Asembia, Excel, GRx, etc.)
3. Communicates to the stakeholders the necessary information to obtain benefit information, authorization, reauthorization, and reimbursement by emailing and scanning pertinent medical information.
4. Contact patient to collect information necessary to establish his/her medical chart. Discuss financial obligations with the new patient.
5. Address insurance issues that require alternate funding. Register and/or open patients with manufacturer reimbursement programs, as needed.
6. Follows-up on authorizations to ensure they are achieved prior to dispensing.
7. Schedule dispensing calendar for new or refill customers.
8. Sets up shipping arrangements as needed.
9. Participate in the Performance Improvement Process.
10. Assist in co-pay collections, which require communication with the patient.
11. Review dispensing calendar and assists with the completion of all order entries and shipments each day.
12. Gather medications and supplies, labels, and has shipments verified for correctness by the Pharmacist.
13. Assist the dispensing technicians as needed regarding required refill requests that are discovered at time of dispensing.
14. Operate POS and rings sales accurately.
15. Follow-up on any shipping exceptions to ensure patient is aware of any possible delays.
16. Assist in ordering medications and supplies, audit receipt of goods, reviews invoices to ensure correctness and routes to appropriate contact.
17. Assist in compiling necessary reports and/or audit information.
18. File as necessary.
19. Assist in maintaining cleanliness and orderliness of the pharmacy.
20. Maintain accurate and complete patient records.
21. Participate in pharmacy orientation programs, training programs, pharmacy staff meetings, in-services, and other programs as required.
22. Exemplify excellence in customer communications and service standards.
23. Maintain good understanding and working knowledge of all VytlOne policies and procedures, and HIPAA operational guidelines, Accreditation requirements, Medicare requirements and other contract guidelines.
24. Maintain good inter/intra departmental relationships. Works to create an amiable atmosphere within the department and company.
25. Respond to change, performance improvement support, professional growth, and met development goals.
26. Promptly report any allegations of impropriety to the Compliance Department.
27. Comply with VytlOne ’s Ethical Business Conduct policy and VytlOne ’s Compliance Program.
28. Remain free from exclusion under the OIG and SAMS Medicare/Medicaid lists.
29. Complete required training, as assigned, within the established timeframes.
30. Must be able to cope with the mental and emotional stress of the position.
31. Maintain regular attendance in accordance with established policies.
32. Perform other job-related duties as assigned.
