    Job Description: Patient Care Coordinator - Unity Health, Parkside

     

    DEPARTMENT: Pharmacy 

     

    REPORTS TO:  Pharmacy Manager and Regional VP of Operations; May also receive additional supervision from the Executive and Divisional VP of Operations.

    Responsibilities

     

    This position carries the responsibility to ensure pharmacy technician skills and customer service is achieved in a professional manner in accordance with current Federal, State, local standards, guidelines, and regulations.

     

    ESSENTIAL FUNCTIONS (This document in no way states or implies that these are the only duties to be performed by the employee occupying this position):

     

    1. Meet performance standards and expectations. Follow all Company policies and procedures.
    2. Assist with projects as necessary.
    3. Assist other areas as needed.
    4. Answers new referral intake and/or refill calls in a timely and professional manner.
    5. Accurately inputs new referrals into the computer system and updates information in the computer system (Asembia, Excel, GRx, etc).
    6. Communicates to the stakeholders the necessary information to obtain benefit information, authorization, reauthorization, and reimbursement by emailing and scanning pertinent medical information.
    7. Contacts patient to collect information necessary to establish his/her medical chart. Discuss financial obligations with the new patient.
    8. Addresses insurance issues that require alternate funding. Registers and/or opens patients with manufacturer reimbursement programs, as needed.
    9. Follows-up on authorizations to ensure they are achieved prior to dispensing.
    10. Coordinates with referrals source all necessary information needed to initiate services for new patient referrals.
    11. Schedules dispensing calendar for new or refill customers.
    12. Sets up shipping arrangements as needed.
    13. Coordinates injection training, if applicable.
    14. Participates in the Performance Improvement Process.
    15. Assists in co-pay collections, which require communication with the patient.
    16. Works cooperatively with others in all departments.
    17. Reviews dispensing calendar and assists with the completion of all order entries and shipments each day.
    18. Enters new referral prescriptions accurately into computer and has Pharmacist verify correctness of the prescription entry.
    19. Gathers medications and supplies, labels, and has shipments verified for correctness by the Pharmacist.
    20. Assists the dispense technicians as needed regarding required refill requests that are discovered at time of dispensing.
    21. Operates POS and rings sales accurately.
    22. Follows-up on any shipping exceptions to ensure patient is aware of any possible delays.
    23. Assists in ordering medications and supplies, audit receipt of goods, reviews invoices to ensure correctness and routes to appropriate contact.
    24. Assists in compiling necessary reports and/or audit information.
    25. Files as necessary.
    26. Assists in maintaining cleanliness and orderliness of the pharmacy.
    27. Maintains accurate and complete patient records.
    28. Participates in pharmacy orientation programs, training programs, pharmacy staff meetings, in-services, and other programs as required.
    29. Exemplifies excellence in customer communications and service standards.
    30. Has a good understanding and working knowledge of all Maxor policies and procedures, and HIPAA operational guidelines, Accreditation requirements, Medicare requirements and other contract guidelines.
    31. May be included in after hour’s on-call rotation.
    32. Completes other miscellaneous duties as assigned.
    33. Maintains good inter/intra departmental relationships. Works to create an amiable atmosphere within the department and company.
    34. Responds to change, performance improvement support, professional growth, and met development goals.
    35. Attendance - Faithfully comes to work daily and conforms to scheduled work hours.
    36. Promptly report any allegations of impropriety to the Compliance Department.
    37. Comply with Maxor’s Ethical Business Conduct policy and Maxor’s Compliance Program.
    38. Remain free from exclusion under the OIG and SAMS Medicare/Medicaid lists.
    39. Complete required training, as assigned, within the established timeframes.
    40. Must be able to cope with the mental and emotional stress of the position. 

      

    Qualifications

    QUALIFICATIONS: Current, Unrestricted DC Pharmacy Technician license
    Education: High School Diploma or equivalency.  National and/or State Certified Pharmacy Technician license, depending on requirements by location.
    Experience: Previous pharmacy experience and customer service experience preferred. 
    Knowledge, Skills, and Abilities:
    1. Working knowledge of computer software, specifically Microsoft Word
    2. Ability to accurately enter data into computer
    3. Ability to prioritize work to meet deadlines
    4. Ability to work accurately under pressure
    5. Ability to work through and solve problems independently
    6. Positive attitude, innovative, self-starter
    7. Excellent organizational and communication skills, both written and verbal
